Health Home 

Yeled v’Yalda’s Health Home program is designed to organize and manage healthcare needs by building and maintaining a connected network of providers and professionals.  Through the Health Home program, Yeled v’Yalda assigns a care manager who provides assistance and support by collaborating with parents to increase access to services and resources, and by helping families develop an all-encompassing and well-organized plan of care. The Health Home program’s goal is to coordinate the full range of healthcare-related matters such as scheduling appointments with providers, organizing transport to and from appointments, maximizing access to available resources, and facilitating effective communication with doctors, hospitals, and other professionals.

HCBS 

Home and Community Based Services are designed for children who are medically fragile or who display imminent risk factors due to their mental health struggles. Services include CFASS, Respite, Pre-vocational Services, Community Habilitation.


HCBS are designed to allow children/youth to participate in developmentally- and culturally-appropriate services through Medicaid. HCBS are intended to provide service options for individuals who would otherwise require care that is provided in a more restrictive environment, such as a long-term care facility or psychiatric inpatient placement, as well as for individuals at risk of requiring that level of care. The array of services offered by HCBS are provided in the home and within the community, and are intended to assist children/youth and families succeed and thrive.

Caregiver Family Advocacy Supports Services (CFASS) 

Caregiver/Family Advocacy and Support Services help children and youth—regardless of their disability (developmental, physical, or behavioral)—function better within their family and community. These services support caregivers and families by providing them with strategies and information to meet the child’s needs, especially when dealing with disabilities.

The goal is to help caregivers and families:

  • Understand the child’s disability and how it impacts daily life.

  • Improve the child’s ability to participate in community activities.

  • Strengthen the family’s ability to care for the child at home and in public settings.

These services can prevent problems by addressing challenges before they become bigger issues. They also help resolve existing concerns related to the child’s disability. However, these services do not cover the development of an Individualized Education Program (IEP) or special education services.

What Caregiver/Family Advocacy and Support Services Offer:

  1. Education and Support

    • Provide caregivers and families with information on available community resources and how to use them effectively.

    • Train caregivers and families to become more self-sufficient in supporting the child at home and in the community.

  2. Family and Caregiver Empowerment

    • Help caregivers understand the child’s condition, particularly chronic or life-threatening illnesses.

    • Guide families through managing the child’s needs and advocating for services.

  3. Community Engagement

    • Offer training to help children/youth and their families succeed in community settings.

    • Provide self-advocacy training, teaching the child and family how to communicate their needs, particularly during community transitions.

By empowering caregivers and families with knowledge and skills, these services help create a supportive environment where children and youth can thrive at home and in their community.

Respite

Respite care is a short-term service designed to provide relief to family caregivers of children and youth, including those with developmental, physical, or behavioral disabilities. It can be planned in advance or provided during a crisis situation.

This service focuses on giving caregivers a break by ensuring the child or youth is supervised and engaged in appropriate activities. The goal is to support the child’s needs while maintaining their health, safety, and development. Respite care providers are trained to understand and implement interventions that help children/youth function at their best, and they work to ensure continuity with the child’s overall care plan.

Respite care may take place in a variety of settings, such as the child’s home, community-based locations, or other approved facilities, depending on the needs of the child and family.

ABA Insurance Services

Special Education & Disability Services Yeled v’Yalda ABA Insurance program services which are approved and authorized by the patient’s private health insurance carrier. Services provided to address the developmental skills necessary for assisted daily living skills in the home and community settings. Early Intervention Services

At Yeled v’Yalda we know that healthy growth and development begins at birth, as infants start discovering and interacting with the world around them. Reaching crucial milestones, in areas such as feeding, movement, and communication, allows children to progress smoothly from one stage to the next along their developmental path. When a child does not achieve certain milestones on time, quick intervention is key to helping get them back on track and avoid falling behind in their skills and abilities. At Yeled v’Yalda, we believe that Early Intervention is one of the most powerful and effective tools in our repertoire. Our Early Intervention (0-3) program addresses the needs of our youngest clients, with the steadfast goal of providing the skills they need to set their development on course as promptly and smoothly as possible. When a parent has concerns about their child’s development, the earlier they seek help, the greater the chance their child is given to overcome the developmental challenges they are facing.

Therapy Referrals

Yeled v’Yalda provides counseling services to promote students’ personal and social development in a safe and inclusive learning environment. When a child is experiencing significant emotional or behavioral difficulties in school, Yeled v’Yalda works closely with the child, family, and school to maximize the benefits of school-based counseling. Yeled v’Yalda’s school-based services are designed to provide the best possible care by ensuring that our counseling providers work and communicate directly with classroom teachers to coordinate the child’s therapeutic goals and objectives.
For many families, attending a traditional outpatient clinic can be challenging. Yeled v’Yalda’s counseling program enables children to receive direct counseling in the school they attend daily. Because our services are school-based, the therapist can (with parent and/or child consent) work closely with the school’s teachers, counselors, personnel, and other staff to help meet the student’s needs.
Aside from the location in which they are provided, school-based counseling services are nearly identical in every way to traditional office-based mental health therapy. At Yeled v’Yalda, our vision is to empower and inspire change in each student we work with. Yeled v’Yalda guides children in rallying their strengths and capabilities and in discovering new potential to overcome challenges and reach their goals.

SEIT Referrals

At Yeled v’Yalda we believe that children learn and develop best in natural, inclusive, and innovative settings.  We recognize that the academic development of children is fundamentally dependent upon the nature of their learning environment.  Yeled v’Yalda strives to provide contemporary, mainstream educational opportunities which are compatible with the specific needs of each child.  Our goal is to help schools, teachers, and parents create well-designed and personalized learning environments in which children’s growth and development can flourish.

Yeled v’Yalda’s SEIT program is designed to focus on the school-readiness skills of preschool children, ages 3-5.  Through our SEIT program, our highly-trained staff of professionals help children learn, and help teachers design and adjust teaching methods to meet each child’s individualized needs and goals.  It is our belief that providing children with early opportunities to develop the skills they need in school will help to create a foundation for academic and learning success as they grow.
